Community Health Outreach Programs
Holy Cross Hospital’s Community Wellness Program provides education, resources and support to individuals in the Taos community who are challenged with the diagnosis or prevention of chronic disease. Assuring high-quality education for patient self-care is one of the primary goals of the Community Wellness Program.
The programs offered by HCH Community Wellness include an ADA-Recognized Diabetes Self-Management Program, Medical Nutrition Therapy, Weight Loss and Fitness Education, Stress Management, Heart-Healthy Living, Diabetes Cooking Classes, and Youth Diabetes Prevention Programs.
The Community Wellness Program is staffed by a nurse and Dietitian Educators and an Administrative Assistant .
