Holy Cross Hospital’s Community Wellness Program provides quality education, resources and support to individuals in the Taos and Peņasco communities who are challenged with the diagnosis or prevention of chronic disease. A primary goal of the Community Wellness Program is to empower the individual with the knowledge, support and skills for self-care. Two nurse educators, a registered dietitian, and an administrative assistant staff the Community Wellness Program. All educators maintain the national CDE (Certified Diabetes Educator) credential, and work closely with the individual’s primary care physician to achieve collaborative health goals.
Access to the ADA-Recognized Diabetes Self-Management Education Program is through physician referral. Education on all aspects of diabetes including disease process, medications, physical activity, blood glucose monitoring, acute and chronic complications, psychosocial adjustment, goal setting and problem solving is included. Intensive insulin management can be provided, and insulin pump starts are facilitated.
A Weight Loss Support Group facilitated by a registered dietitian meets weekly (at no charge). The Weight Loss Support Group utilizes the expertise of a Certified Personal Fitness Trainer and a Licensed Professional Counselor for stress management approaches to successful weight loss.
A Diabetes Cooking Class is held once a year and consists of 4 group sessions. The Pre-Diabetes Class is held quarterly and is aimed at the prevention of Type 2 Diabetes in children and adults.
The Cancer Support Group meets monthly and is facilitated by a licensed social worker. This group is open to people with cancer, their family and friends, as a place to come together and share feelings, support and information. Please call (575) 751-5750 for more information.
